Making an Appointment
As a service to our patients, we are happy to file your insurance claims. However when calling for an appointment, be prepared to provide insurance information including your driver's license number and social security number, and the same information of the policy holder(if different from your own). The coordinator will confirm benefits before your appointment and will contact you with any concerns.

Otherwise, we will provide you with a bill complete with codes so that you may file your own claim with your insurance.

  Note: We take all major insurance and Medicare referrals.
