Making an Appointment
Meet our benefits coordinator to help you with confusing details of increasingly complex insurance benefits.    When calling for an appointment, please provide insurance information including your driver's license number and social security number, and the same information of the policy holder (if different from your own).  The coordinator will confirm benefits before your appointment and will contact you with any concerns.   Note: We take all major insurance and Medicare referrals.
